It is important to know that Smart Health Book is NOT insurance. It is a coordination service that includes multiple high-value options for accessing healthcare. It also includes a discount dental plan that provides significant savings on dental procedures for you and your family. It is an easier way to save money in your dental health. Smart Health Book gives you access to quality dental care at rates that are usually exclusive to expensive insurance plans.
Discount dental plans are an affordable and easy-to-use alternative to dental insurance. Plan members have access to a large network of participating dentists that have agreed to offer their services at significantly discounted contracted rates.
You are paying for access to our coordinators, mobile application, and access to various discounted health services including, a discount dental plan, vision discounts, pharmacy discounts…etc. Essentially you are paying for a plan that will save you money on your healthcare services.
The Smart Health Book mobile app allows anyone to browse our network of participating dentists and other providers by specific specialty, service, zip code, and distance.
With a paid membership in Smart Health Book members are able to search our network of participating dentists and other providers by specific specialty, service, zip code, and distance. Paying members also have access to the discounted rates per provider for specific services, the savings amount for selected services (if applicable), and the ability to request an appointment for with a specific provider right through the application. Paying members receive notifications and appointment confirmations right through the application. A paid membership includes so much more!

With either membership level you can enjoy all of the valuable features of Smart Health Book. An individual membership is for one person, while a Family membership allows up to 6 total members in the same household.
When you first enroll, you will receive a Welcome Email. Your membership is active as of the “Active Date” specified in the Welcome Email.
Requesting an appointment is simple. First, search for a provider in the Smart Health Book mobile app or website. Select the provider you want to see and click on the “Book Me!” button. Follow the prompts to enter your appointment details and preferences. Once you have completed entering all the details for your appointment request, click on the “BOOK MY APPOINTMENT” button. This will submit your request to the provider and our Elite Care Coordination department. You will receive a notification in the app that shows your pending appointment request. Once your appointment is confirmed, you will receive an Appointment Confirmation notification and email with the confirmed date and time of your appointment. It’s that easy!

If you need to cancel your appointment, you MUST call the provider directly and cancel with their office. As always, if you need any assistance, do not hesitate to contact us directly.
Here at Smart Health Book, we strive to satisfy all of our member’s requests, however, there are specific requests that may not be able to be met. For example, we cannot guarantee that a particular provider will have availability for new appointments within a specific time frame. In order to achieve the best results, we recommend that you submit your request with as much flexibility in your requirements as possible (For example: The days and times you are available for appointments, or allowing us to schedule you with an alternate provider that meets your criteria…etc.). This will give us the best chance to meet your needs and get your appointment confirmed.
Your membership in Smart Health Book includes access to well over 200,000 dental providers nationwide. Unfortunately, there may be specific providers who do not participate in the large national networks, or they may not know that they are contracted with the network. Please call us if you have questions or need more information about the dentists in your area who do accept your dental savings plan.
Yes, you can cancel your membership at any time, but why would you? You are getting an exceptional value from your Smart Health Book Membership! However, we want you to be totally satisfied, so, if you cancel within the first 30 days, you are eligible to receive a full refund, of your membership fee excluding the one-time application fee. You can only use one membership at a time, however, if you have family members who do not live in your household, they will need to purchase a separate membership (certain restrictions or exceptions may apply).
Contact us directly to get assistance with financing for more costly procedures. We will help you apply for financing from one of our 3rd party partners who can assist almost anyone; even those with less-than-perfect credit.
